This package builds upon The Cornerstone by adding more comprehensive health services and specialist coordination while maintaining the DPC model.
SERVICES INCLUDE
All services from The Cornerstone package.
Specialist Coordination: Referrals and direct coordination with specialists (e.g., cardiologists, dermatologists) for advanced care, though specialist visits are arranged outside of the DPC network.
Expanded Lab Work: Additional non-imaging diagnostics and lab tests, with coordination for more in-depth assessments through external labs.
Mental Health Services: Access to in-house or partnered mental health providers for counseling, therapy, and psychiatric support.
Outpatient Procedures: Coverage for minor outpatient procedures (e.g., skin biopsies, minor suturing) performed in the DPC setting.
Rehabilitation Services Coordination: Physical therapy, occupational therapy, and speech therapy referrals and coordination, while remaining within the outpatient scope of care.
Prescription Management: Expanded access to wholesale medications, including select brand-name prescriptions.
Preventive Screenings (Non-Imaging): Access to advanced screenings, including hormone level testing, genetic predispositions, and advanced blood work.
