Concierge
Membership Fees
Concierge care allows patients to enjoy unfettered access to all Cornerstone Health membership benefits while still utilizing their commercial insurance plan to cover the fees of service. Concierge patients pay the membership fees outlined below, and we file their commercial insurance for any medical services provided. Please note: concierge patients are still responsible for meeting their insurance deductible and paying any required co-payments. This is required by the terms of our contracts with commercial insurance providers. We currently take all commercial insurance as well as Medicare and TriCare.

Upon joining Cornerstone Health, we will collect a one-time, non-refundable Enrollment Fee of $60 per patient not to exceed a family maximum of $150. This fee is separate from your first month’s payment.
Individual Fees
Members may opt for monthly billing or annual billing with a 5% discount.

*Children: The first two children are at the listed rate above. The third child is $15. There is no charge for any children past the third child.
